Symptoms and Signs – Differential Diagnosis of Abdominal Pain

Get ready to place a nasogastric tube to alleviate sudden swelling inside the digestive tract. Provide the patient with reassurance and adequately prepare them for the surgical procedure.
Historical and Physical Assessment
To determine the cause of the patient's abdominal distention, inquire about the time of its occurrence, how long it has been there, and any accompanying symptoms. A patient experiencing localized distention may describe feeling pressure, fullness, or discomfort in the affected location. A patient experiencing global distention may describe sensations of bloating, palpitations, and dyspnea during deep breathing or when in a supine position.
The patient may also experience a lack of flexibility in his waist region. Ensure to inquire about symptoms such as abdominal pain, fever, nausea, vomiting, loss of appetite, changes in bowel movements, and fluctuations in body weight.
Collect a comprehensive medical record, paying attention to gastrointestinal (GI) or biliary problems that could potentially lead to peritonitis or ascites, such as cirrhosis, hepatitis, or inflammatory bowel disease. Additionally, please take notice of the presence of chronic constipation. Has the patient undergone recent abdominal surgery, resulting in abdominal distention? Inquire about recent incidents, including small ones, such as experiencing a mishap while descending from a stepladder.
Conduct a comprehensive physical examination. It is vital not to limit the examination to the abdomen as this may result in overlooking significant indicators of the underlying etiology of abdominal distention. Then, position yourself at the bottom of the bed and carefully examine the patient who is lying down to check for any differences in the shape or size of their abdomen. This will help you establish if the swelling is limited to a specific area or if it is spread across the abdomen. Next, evaluate the shape of the abdomen by bending down at his side. Examine the skin for tightness and protruding sides, as these could be signs of ascites. Examine the umbilicus. An outwardly turned belly button may suggest the presence of ascites or an umbilical hernia. A concave navel can be a sign of abdominal bloating caused by gas accumulation; it is also frequently observed in individuals who are obese. Examine the abdominal area for indications of inguinal or femoral hernia, as well as wounds that could indicate the presence of adhesions. Both can result in gastrointestinal blockage. Next, listen for bowel sounds, abdominal friction rubs (which suggest inflammation of the peritoneum), and bruits (which indicate the presence of an aneurysm). Pay attention to the succussion splash, which is a splashing sound typically heard in the stomach when the patient moves or when palpation disrupts the internal organs. Identifying Ascites
In order to distinguish ascites from other factors that may produce abdominal distention, it is important to assess for shifting dullness, fluid wave, and puddle sign, as outlined in this description.
Alteration of dullness
Step 1. Position the patient in a lying down position, facing upwards. Use percussion to strike the area from the belly button outward towards the side of the body, as demonstrated. Create a straight mark on the patient's skin to indicate the transition from a resonant sound to a less resonant sound.
Step 2. Rotate the patient to a lateral position. (Please be aware that this placement results in the movement of ascitic fluid.) Re-percuss and observe the transition from a resonant sound to a less resonant sound. An observed disparity between these lines may suggest the presence of ascites.
Fluid wave
Instruct another individual to apply firm pressure to the patient's midline in order to inhibit the transmission of vibrations along the abdominal wall. Position one of your palms on the patient's flank. Execute a forceful blow to the other side with your non-dominant hand. If you perceive impact on the palm that is opposite to the one being struck, it indicates the presence of ascitic fluid.
Puddle Sign
Position the patient on his elbows and knees, which causes ascitic fluid to pool in the most dependent part of the abdomen. Perform percussion on the belly, starting from the side and moving towards the center. The percussive sound intensifies around the periphery of the puddle, also known as the ascitic pool.
Subsequently, employ percussion and palpation techniques on the abdomen to ascertain if the distention is caused by the presence of air, fluid, or a combination of both. An air-filled descending or sigmoid colon is indicated by a tympanic tone in the left lower quadrant. An air-filled peritoneal cavity is indicated by a tympanic note heard across a broadly enlarged abdomen. A monotonous pounding sound throughout an overall swollen belly indicates the presence of fluid in the peritoneal cavity. Lateral displacement of dullness while the patient is in the decubitus posture is also indicative of an abdominal cavity filled with fluid. A pelvic or intra-abdominal mass will result in a lack of resonance when percussed and should be able to be felt upon palpation. Obesity leads to a significant increase in abdominal size without any noticeable change in dullness when tapping on the abdomen, a conspicuous hollow sound when tapping, or the ability to feel any masses in the bowels or elsewhere, with a general sense of dullness rather than a specific localized area.
Examine the belly by applying pressure to detect any pain, and observe if the pain is exclusive to a certain area or spread over the entire abdomen. Be vigilant for indications and symptoms related to the peritoneum, such as rebound soreness, guarding, rigidity, McBurney's point, obturator sign, and psoas sign. Female patients should receive a pelvic examination, while male patients should receive a genital examination. It is recommended that all patients who complain of abdominal pain should receive a digital rectal examination along with fecal occult blood tests. Lastly, obtain the patient's abdomen circumference as a reference value. Use a felt-tipped pen to mark the sides as a point of reference for future measurements.
Underlying physiological factors that contribute to a medical condition.
Gastrointestinal malignancy.
Abdominal distention can occur when cancer, typically ovarian, hepatic, or pancreatic, causes the accumulation of fluid in the abdomen, known as ascites. This is more likely to happen in patients who already have a tumor. It is a sign of a progressed illness. The presence of shifting dullness and a fluid wave is observed along with distention. Common indications and symptoms may comprise of intense stomach discomfort, a mass in the abdomen, lack of appetite, yellowing of the skin and eyes, gastrointestinal bleeding (vomiting blood or dark stools), indigestion, and gradual weight loss leading to muscle weakness and wasting.
Abdominal injury
When there is rapid internal bleeding that occurs with trauma, the abdomen may become swollen and distended in a sudden and striking manner. The indicative manifestations of this potentially fatal condition encompass abdominal stiffness accompanied by protective muscle contractions, reduced or nonexistent intestinal noises, emesis, sensitivity to touch, and discoloration of the abdominal area. Discomfort may manifest either at the location of the injury or at the scapula if bleeding in the abdomen causes irritation to the phrenic nerve. Hypovolemic shock manifests with notable symptoms, including low blood pressure and a fast, weak pulse, when there is substantial blood loss.
Cirrhosis
Cirrhosis is a medical condition characterized by the scarring of the liver tissue. Ascites in cirrhosis leads to widespread swelling and can be confirmed by the presence of a fluid wave, fluctuating dullness, and a puddle sign. Umbilical
Eversion and caput medusae, which refers to the dilation of veins around the umbilicus, are frequently observed. The patient may have sensations of satiety or an increase in body mass. Additional symptoms may include nonspecific abdominal discomfort, elevated body temperature, loss of appetite, feelings of sickness, vomiting, difficulty with bowel movements, abnormal bleeding, intense itching, reddening of the palms, spider-like blood vessels, swelling in the legs, and even an enlarged spleen. Additionally, one may observe hematemesis, encephalopathy, gynecomastia, or testicular shrinkage. Jaundice typically manifests as a late indication. Initially, hepatomegaly is seen, but in severe disease, the liver may not be palpable.
Cardiac Failure
. Severe cardiovascular impairment often leads to generalized abdominal distention caused by ascites. This condition can be validated by observing fluctuating dullness and a fluid wave. The manifestations and indications of heart failure are abundant and vary based on the stage of the disease and the extent of circulatory dysfunction. Characteristic signs of this condition include swelling in the extremities, visible distention of the jugular vein, difficulty breathing, and a rapid heart rate. Typical indications and symptoms of this condition include an enlarged liver (which can cause pain in the upper right part of the abdomen), feelings of nausea, vomiting, a cough that produces phlegm, crackling sounds in the lungs, cold extremities, bluish nail beds, increased urination at night, difficulty exercising, wheezing at night, high blood pressure during the relaxation phase of the heartbeat, and an enlarged heart.
Irritable bowel syndrome (IBS).
Irritable bowel syndrome can cause sporadic, localized distention, which occurs due to recurrent contractions of the intestines. These spasms are usually accompanied by lower abdomen pain or cramps. The pain is typically alleviated by bowel movements or the release of intestinal gas and is exacerbated by stress. Additional potential indications and manifestations encompass irregular bowel movements characterized by alternating episodes of diarrhea, constipation, or regular bowel function, feelings of nausea, indigestion, exertion and urgency during defecation, a sensation of incomplete emptying, and the presence of tiny stools stained with mucus.
Large Bowel Obstuction
This life-threatening condition is characterized by dramatic abdominal distention, to the extent that loops of the large bowel may become visible on the abdomen. Constipation occurs before distention and can be the sole symptom for several days. Additional observations include the presence of a hollow sound when tapping the abdomen, increased frequency of high-pitched noises produced by the intestines, and the abrupt appearance of cramp-like pain in the lower abdomen that continues without relief. Regurgitation of fecal matter and reduced muscular contractions and noises in the intestines are indications that appear in the later stages.
Acute mesenteric artery occlusion.
In this potentially life-threatening condition, the abdomen becomes swollen usually a few hours after the sudden start of intense, cramp-like pain around the belly button, accompanied by quick (and often strong) bowel movements. The pain subsequently becomes persistent and widespread. Signs and symptoms that are associated with this condition include intense pain and sensitivity in the abdomen, stiffness and tension in the muscles, lack of bowel noises, and sometimes a vascular sound in the right lower abdomen fossa
. The patient may also have emesis, anorexia, enteritis, or gastrointestinal stasis. Delayed symptoms encompass elevated body temperature, rapid heart rate, accelerated breathing, low blood pressure, and cold, moist skin. If there is no discomfort, abdominal distention or gastrointestinal bleeding may be the only indicators.
Paralytic ileus
Paralytic ileus is a medical condition characterized by the paralysis of the intestines. Paralytic ileus is a condition characterized by widespread swelling and a hollow sound when tapped. It is followed by a lack of or reduced bowel noises, and sometimes minor abdominal pain and vomiting. The patient may experience either severe constipation or the passage of flatus and tiny, liquid stools.
Peritonitis.
Peritonitis is a potentially fatal condition characterized by inflammation in the abdomen, which can cause either localized or widespread abdominal distention depending on the severity of the infection. Fluid collects in the peritoneal cavity and subsequently in the bowel lumen, resulting in a fluid wave and a change in sound when tapping on the abdomen. Distention is usually accompanied with abrupt and intense stomach discomfort that increases with movement, rebound tenderness, and abdominal rigidity. The patient's abdominal skin may exhibit a taut appearance. Typical indications and symptoms often involve reduced or missing bowel noises, elevated body temperature, shivering, heightened sensitivity to pain, feeling sick, and throwing up. Tachycardia and hypotension are observed as indications of shock when there is a substantial loss of fluid into the abdomen.
Intestinal blockage in the small bowel.
Abdominal distention is a prominent feature of small-bowel blockage, a dangerous condition that can be life-threatening. This distention is particularly noticeable in the later stages of obstruction, especially in the lower part of the small intestine. During auscultation, one may see either reduced or increased bowel sounds, while percussion of the abdomen generates a resonant sound. Additional indications and symptoms that may be present include intermittent abdominal pain around the belly button, difficulty passing stool, feelings of sickness, and throwing up; the severity and timing of the vomiting tends to increase as the blockage moves higher in the digestive system. Rebound tenderness indicates the presence of intestinal constriction accompanied by reduced blood supply. Common indications and manifestations encompass fatigue, general discomfort, and indications of insufficient hydration. The manifestation of hypovolemic shock becomes evident as dehydration and loss of plasma worsen.
Acute toxic megacolon.
Toxic megacolon is a potentially fatal complication that can arise from either infectious or ulcerative colitis. Providing guidance and advice to patients
Instruct the patient on the technique of slow breathing as a means to alleviate stomach discomfort, and stress the significance of maintaining good dental hygiene to prevent the occurrence of dry mouth. Inform the patient about the specific meals and fluids to be avoided in the presence of a blockage or ascites.
Tips for Pediatrics
Due to the natural roundness of a young child's abdomen, it might be challenging to notice distention. Thankfully, the abdominal wall of a youngster is less developed compared to an adult, which makes it easier to feel and examine by touch. When performing percussion on the belly, it is important to keep in mind that children typically ingest air while feeding and crying, which leads to an increased resonance known as tympany. Abdominal distention, accompanied by little tympany, can occur due to the buildup of fluid or the presence of solid masses. Undress the child completely to ensure that no diagnostic clues are overlooked. Additionally, conduct a mild rectal examination.
Neonates typically develop ascites as a result of gastrointestinal or urinary perforation, while older children commonly experience ascites due to heart failure, cirrhosis, or nephrosis. In addition to ascites, abdominal distention can be caused by congenital anomalies of the gastrointestinal tract, such as intussusception and volvulus. If a hernia results in an intestinal obstruction, it can lead to distention. Furthermore, excessive consumption of food and difficulty in passing stool might result in abdominal distention.
Pointers for the Elderly
As individuals grow older, adipose tissue has a tendency to build in the lower abdominal region and around the hips, regardless of whether body weight remains constant.
