• Drug induced (especially angiotensin-converting enzyme [ACE] inhibitors, b-blockers)
• Exposure to irritants (noxious fumes, smoke, cold air)
• Gastroesophageal reflux disease (GERD)
• Infectious process (viral, bacterial)
• Inflammation of larynx, pleura, diaphragm, mediastinum
• Interstitial lung disease
