Symptoms and Signs – Differential Diagnosis of Abdominal Rigidity

Abdominal rigidity is the term used to describe the aberrant muscle tension or inflexibility of the abdomen, which can be detected through palpation. Rigidity can be either voluntary or involuntary. Voluntary rigidity indicates that the patient is consciously tensing their muscles due to anxiety or anxiousness while being touched; involuntary rigidity indicates a potentially life-threatening condition of irritation or inflammation in the peritoneum. Involuntary rigidity primarily arises from gastrointestinal illnesses, however it can also stem from pulmonary and vascular disorders, as well as the impact of bug poisons. Typically, it is accompanied by fever, nausea, vomiting, and abdominal tenderness, distention, and pain.

Emergency interventions

Historical and Physical Assessment
If the patient's condition permits more evaluation, obtain a concise medical history. Determine the onset of abdominal stiffness. Does it correlate with abdominal pain? Did the pain commence simultaneously, if so? Ascertain whether the abdominal rigidity is confined to a specific area or present throughout the entire abdomen. Is it consistently present? Has the location of the site changed or remained consistent? Subsequently, inquire about variables that may worsen or relieve the symptoms, such as changes in position, coughing, vomiting, bowel movements, and walking.

Investigate additional indications and manifestations.
Examine the abdominal region for peristaltic waves, which may be observable in individuals with a very low body fat percentage. Additionally, please examine for any noticeable enlargement of the intestinal loop. Next, listen to the sounds produced by the intestines. Differential Diagnosis of Abdominal Rigidity
Dissecting abdominal aortic aneurysm. Abdominal aortic aneurysm, a dangerous condition, causes mild to moderate stiffness in the abdomen. Usually, it is accompanied by persistent upper abdominal pain that may extend to the lower back. The pain may intensify when the patient reclines and may be alleviated when he inclines forward or assumes an upright position. Prior to rupture, the aneurysm may cause a throbbing lump in the upper abdomen, accompanied by a noise during the contraction phase of the heartbeat over the aorta. Nevertheless, the bulk ceases to pulsate after it ruptures. Common indications and manifestations include discolored skin below the waist, lack of femoral and pedal pulses, lower blood pressure in the legs compared to the arms, and mild to moderate sensitivity with protective muscle contractions. Significant hemorrhage results in symptoms of shock, including rapid heart rate (tachycardia), rapid breathing (tachypnea), and cold, moist skin.

Toxins produced by insects.

Rigidity manifests in the central or periumbilical area and is accompanied by intense abdominal discomfort, fever, and indications of shock, such as rapid heart rate and low blood pressure. Additional observations may encompass symptoms such as emesis, loss of appetite, and gastrointestinal irregularities such as diarrhea or constipation. Always consider this illness as a potential diagnosis in individuals who are above the age of 50 and have a medical history that includes heart failure, arrhythmia, cardiovascular infarction, or hypotension.
Peritonitis. The presence of peritonitis can result in either localized or generalized abdominal rigidity, depending on its underlying cause. For instance, if an infected appendix leads to inflammation of the peritoneum in a specific area, rigidity may be concentrated in the lower right quadrant. In cases when a ruptured ulcer leads to extensive peritonitis, the rigidity of the abdominal muscles may become widespread and, in severe instances, resemble a rigid board.

Peritonitis
Peritonitis also induces abrupt and intense stomach pain that might be confined to a specific area or spread throughout the abdomen. Furthermore, it can cause sensitivity and swelling in the abdomen, increased pain when pressure is released, protective muscle contractions, heightened sensitivity to pain, decreased or missing bowel sounds, feelings of nausea, and the act of expelling stomach contents. Typically, the patient also exhibits symptoms such as elevated body temperature, shivering, rapid heart rate, rapid breathing, and low blood pressure.
Unique factors to take into account
Please maintain vigilant observation of the patient for any indications of shock. Ensure his positioning is optimized for maximum comfort. The patient should assume a supine position, with his head in a horizontal position on the table, arms positioned alongside the body, and legs slightly bent to alleviate tension in the abdominal muscles. Refrain from administering analgesics until a preliminary diagnosis has been established, as they may obscure symptoms. In order to potentially perform emergency surgery, it is advisable to refrain from providing any food or water and instead deliver an intravenous antibiotic. Arrange the patient for diagnostic examinations, which may encompass blood, urine, and stool analyses;

The diagnostic procedures include chest and abdominal X-rays, computed tomography scan, magnetic resonance imaging, peritoneal lavage, and gastroscopy or colonoscopy. Additionally, a pelvic or rectal examination may be performed.
