Inquire with the patient about any history of a metallic taste, olfactory impairment, heightened thirst, heartburn, dysphagia, aversion to meals, or nocturnal emesis. Considering the high incidence of gastrointestinal bleeding in individuals with chronic renal failure, inquire about their bowel habits, particularly focusing on the presence of dark stools or constipation.
Record the patient's vital signs. Monitor for signs of hypertension (the patient with advanced chronic renal failure often has moderate hypertension) or hypotension. Monitor for additional indications of shock, such as increased heart rate, rapid breathing, and low body temperature, as well as changes in mental state. Complications such as extensive gastrointestinal bleeding or pericarditis with tamponade can be indicated by significant alterations.

Chronic renal failure in its latter stages
Detection of ammonia breath odor occurs late.
