Cacosmia is the perception of a foul or unpleasant odor without an appropriate external source, occurring as either a hallucination or an illusion. It may be associated with neurological conditions such as temporal lobe epilepsy, sinus disorders, head injury, or certain psychiatric disturbances. Because olfactory perception is closely linked to emotional and memory systems in the brain, disturbances such as cacosmia can be particularly distressing. The term is sometimes spelled kakosmia and is contrasted with related conditions such as anosmia (loss of smell), hyperosmia (heightened smell sensitivity), and parosmia (distorted smell perception).
