The Gemfibrozil ( Fibrates) The presence of fibrates results in a significant reduction in plasma VLDL and, consequently, triglyceride levels; a moderate reduction in LDL-C; and a slight increase in HDL-C. Mode of Action Fibrates stimulate the transcription of genes that are responsible for lipoprotein lipase as well as the apoproteins apoA1 and apoA5, both of which are ligands for certain receptors. LDL-C receptors are able to take in more LDL-C when fibrates are present. The drug is taken orally and is absorbed well; it is metabolized to glucuronide conjugates and then eliminated by the kidney. Use in Clinical Settings Utilized for the treatment of mixed dyslipidemia, which is characterized by a rise in both plasma triglycerides and cholesterol levels. Additionally, it is utilized in cases when HDL levels are insufficient, which therefore raises the risk of atheroma. Adverse Reactions Disturbances of the gastrointestinal tract, a rash, a moderately increased risk of gallstones, and myositis, which can be severe. Avoid taking statins in conjunction with fibrates.
