Therapy

Symptoms and signs 
The individual may have bleeding and blood clot formation, which can be observed through bleeding from locations where a needle has been inserted, little red or purple spots on the skin (petechiae), and reduced blood supply to the fingers and toes (ischemia). Additionally, there may be a condition called microangiopathic hemolytic anemia, which involves the destruction of red blood cells due to abnormal blood vessel function.
Laboratory results: Extended prothrombin time (PT) and partial thromboplastin time (PTT), prolonged bleeding duration, and prolonged thrombin time. elevated D-dimer concentrations, reduced fibrinogen levels, diminished platelet count, decreased hematocrit

Therapy 
Treatment includes platelet transfusion, administration of fresh frozen plasma and cryoprecipitate to restore diminished clotting factors and fibrinogen levels, as well as addressing the underlying illness.
Coagulopathy of liver disease is characterized by prolonged PT (prothrombin time), PTT (partial thromboplastin time), bleeding time, and thrombin time. This condition does not have any connection with small blood clots and can be effectively treated with vitamin K.

Therapy 
Lower blood pressure to a level below 140/90 mm Hg by implementing lifestyle modifications and using antihypertensive medication. Address the root causes of secondary hypertension and administer intravenous antihypertensive therapy in cases of hypertensive emergencies.
Lifestyle modifications: Weight reduction, physical activity, decreased sodium consumption, cessation of smoking, and moderate alcohol consumption.
Pharmacological substances: ACE inhibitors, calcium channel blockers, beta blockers, diuretics, and alpha blockers.

Pathology: Arteriole: Hyperplastic arteriosclerosis is characterized by the concentric, laminated onion skin thickening of arterial walls, accompanied by necrotizing arteriolitis, which involves the deposition of fibrinoid in arteriole walls and subsequent necrosis and inflammation. This condition can lead to other pathological conditions, such as left ventricular hypertrophy and failure, as well as malignant nephrosclerosis, which involves the rupture of glomerular capillaries and the resulting appearance of a flea-bitten kidney.

Medical Presentation
Frequently manifests as a hypertensive crises, characterized by symptoms such as headache, altered mental status, blurred vision, chest pain, or dyspnea.
The physical examination reveals a misplaced and powerful point of maximal impulse at the cardiac apex. Additionally, there is the presence of S4, papilledema, ocular hemorrhages and exudates, and a significant increase in diastolic pressure above 120 mm Hg.

Atheromas, which are fatty deposits in the arterial wall, can become complicated by the formation of blood clots on top, ulceration, or calcification of the plaque. These atheromas are typically seen in elastic arteries and medium to large muscular arteries.

Symptoms and signs 
Frequently lacking symptoms: may manifest as angina or claudication (muscle discomfort during physical activity, alleviated by rest).
Complications including aneurysms, myocardial infarction (MI), stroke, intestinal ischemia, renal artery ischemia, and peripheral vascular occlusive disease. and blood clots that form on top of a thrombus or on the plaque itself

Therapy 
Treatment options for reducing lipid levels include HMG-CoA reductase inhibitors. Additionally, smoking cessation and modifying risk factors such as controlling diabetes and blood pressure are recommended.
Xanthomas manifest as yellowish plaques or nodules on the skin. These structures consist of macrophages filled with lipids and are linked to high levels of cholesterol in the blood.

​Pathology - Aortic Aneurysm
Linked to atherosclerosis, coronary artery disease (CAD), physical injury, genetic risk, and degeneration of the middle layer of arterial walls (cystic medial necrosis)
More commonly observed in males over the age of 50.

Pathology Artery: Typically located in the descending aorta below the renal arteries, although it can also occur in the thoracic aorta. It develops in areas where the arterial wall weakens (such as when atherosclerotic plaque destroys the media layer) or in localized dilations of arteries and veins. These dilations often take the form of saccular outpouchings that are frequently filled with atherosclerotic plaque or thrombus.

Symptoms and signs 
Typically, an abdominal aortic aneurysm (AAA) does not show any symptoms until it bursts. In such cases, it may cause a pulsating, painless mass in the upper abdomen. However, when the AAA ruptures, it leads to intense ripping abdominal agony that spreads to the back and low blood pressure. Individuals afflicted with ascending thoracic aneurysms may experience aortic insufficiency or suffer from chest pain.
Complications may arise from the blockage of renal iliac and/or mesenteric arteries due to the presence of thrombus emboli, or from the occurrence of rupture.
Imaging: Dual-channel lumen of the aorta.

Therapy 

Surgical intervention is recommended for asymptomatic patients with an aneurysm that is rapidly increasing in size and exceeds 5.0 cm. Ruptured aneurysms require prompt surgical repair. Blood pressure should be managed using b-blockers and ACE-inhibitors. Smoking cessation is advised, and lipid-lowering medication should be pursued.
Men aged over 65 with a history of tobacco use are advised to undergo a single screening abdomen ultrasonography to detect any potential abnormalities. 
Tertiary syphilis is linked to the development of syphilitic aneurysm. The blood flow to the aorta (vasa vasorum) is interrupted, leading to the development of an aneurysm in the ascending aorta or aortic arch. This aneurysm has a distinct look resembling the bark of a tree and is associated with aortic valve insufficiency.

​Pathology - Aortic Dissection
Hypertension, trauma, Marfan syndrome, Ehlers-Danlos syndrome, coarctation of the aorta, bicuspid aortic valve, and the last trimester of pregnancy are all linked to this condition. However, there is no connection with atherosclerosis.

Gross Anatomy: There is a tear in the inner layer of the aorta, which has led to the formation of a hematoma within the wall of the aorta. There is also a possibility of the presence of cystic medial necrosis, which is a condition that makes the aorta more prone to dissection, characterized by the separation of the elastic and muscular components of the middle layer of the aorta.

Symptoms and signs 
The individual experiences abrupt and intense left-sided chest discomfort, which frequently spreads to the back. Over time, the pain gradually moves downwards.
One of the complications that can arise is a condition called aortic valve insufficiency. The presence of dissection in a coronary artery or aortic rupture typically results in hemopericardium, cardiac tamponade, and ultimately, fatality.

Therapy
Surgical restoration If the dissection includes the ascending aorta or if there is substantial involvement of branch arteries, it is necessary to maintain rigorous control of heart rate and blood pressure using beta-blockers, ACE inhibitors, and nitroprusside.

Aortic dissections can be categorized as either type A, which involves both the ascending and descending aorta, or type B, which only involves the descending aorta.

Polyarteritis Nodosa is a medical condition characterized by inflammation of the blood vessels.
The cause of this condition is currently unidentified, but it is linked to hepatitis B virus (HBV) infection in 30% to 50% of individuals.
It primarily impacts men in their middle age, but it can also occur in individuals of any age and gender.

Pathology 
Gross Pathology: Impacts muscular arteries of small or medium size, particularly at the junctions of vessels in the kidney, heart, liver, or gastrointestinal tract. The lesions vary in age.
Study of cellular and tissue abnormalities at a microscopic level. The artery wall is experiencing inflammation that affects all layers, with the presence of neutrophils, eosinophils, and mononuclear cells infiltrating the area. Fibrinoid necrosis may be observed.

Clinical manifestations and indications 
The patient presents with fever, unintentional weight loss, general discomfort, abdominal pain accompanied by nausea and vomiting, joint pain, kidney failure, nerve damage, high blood pressure, cotton-wool spots in the retina, inflammation of the heart muscle and the outer lining of the heart, and the presence of small purple spots that may be felt on the skin.
Laboratory results: Increased erythrocyte sedimentation rate (ESR) with elevated white blood cell count (leukocytosis)

Treatment options include the use of corticosteroids, azathioprine, cyclophosphamide, and antiviral treatment for HBV-related conditions.

Churg-Strauss Vasculitis is a medical condition characterized by inflammation of blood vessels. The exact cause of this condition is still unknown. However, it is believed that a certain type of immune system dysfunction has a role in the development of the disease.
It primarily impacts middle-aged males but can manifest in individuals of any age and gender.

Gross Pathology: Impacts muscle arteries and veins of small to medium size, particularly in the lung, kidney, heart, spleen, nerves, skin, or gastrointestinal system.
Microscopic Pathology: Arterial wall inflammation characterized by a preference for lymphocytic (and eosinophilic) infiltration in the outer layer, with the possible presence of fibrinoid necrosis.

Clinical manifestations 
Late-onset asthma is typically characterized by classical symptoms. Sinusitis and rhinitis. The patient exhibits lung involvement characterized by the presence of pulmonary infiltrates, which manifest as a cough. Additionally, the patient also experiences polyneuropathy, specifically mononeuritis multiplex. Additionally, one may see the presence of purpura and arthralgias.
Laboratory results: Increased erythrocyte sedimentation rate (ESR), elevated levels of eosinophils, and p-ANCA positivity observed in 40% of patients.

Treatment: Administering high doses of corticosteroids, specifically cyclophosphamide.

​Pathology - Wegener Granulomatosis 
Attributable to an immunologic mechanism, potentially a hypersensitivity reaction.
Primarily affects males between the ages of 40 and 60, however it can manifest at any age.

Study of the nature and causes of diseases. 
Gross Pathology: Vascular granulomatous lesions observed in the upper respiratory tract, lungs, and kidney.
Study of cellular and tissue abnormalities at a microscopic level. The presence of necrotizing granulomas characterized by a central area of necrosis surrounded by lymphocytes. The presence of macrophages and large cells within the vascular walls, together with granulomatous vasculitis characterized by an inflammatory infiltrate surrounded by fibroplastic growth, is observed. Additionally, there is evidence of rapidly increasing glomerulonephritis in the renal biopsy.

Clinical manifestations and indications 
Nasal septum perforation, chronic sinusitis, otitis media, difficulty breathing, coughing up blood, blood in urine, skin rash, muscle pain, heightened risk of deep vein thrombosis.
Complications encompass renal failure, sometimes known as nephritic syndrome, as well as hearing.
Imaging: Chest X-ray reveals prominent nodular opacities.
Laboratory results: The patient has a positive c-ANCA result and the presence of red blood cells and/or red blood cell casts in their urine.
